コンタクトレンズ販売及び装用体験に際してお客様とのお約束
会員登録することで、紙の同意書への記載が不要になります。
コンタクトレンズ購入ができるのは以下の項目すべてに該当する方に限らせていただきます。
お約束内容をご確認のうえ、同意をしてください。 -
お客様への確認事項
□ 眼科を受診し、指示に従った購入をしている。
□ 定期的に眼科を受診し、医師の指示に従っている。 -
お客様へのお願い
□ 眼科で定期検査を受け、医師の指示に従ってください。
□ 添付文書をよく読んで使用方法、レンズケア、装用時間を守って下さい。
□ 添付文書は必要時に読めるよう保管して下さい。
□ 装用により眼障害が発生する可能性があります。少しでも異常を感じたら使用中止して直ちに眼科を受診して下さい。
□ 当店では瑕疵以外の返品は受けられません。
□ 当店では、コンタクトレンズの装着説明、処方は一切出来ません。眼科医の指導、処方を受けてください。
□ 眼科医の指示や、添付文書の使用方法を守らなかったことで生じた問題は、使用者の責任になります。

Agreement for the Purchase and Trial Use of Contact Lenses
By registering as a member, you will not need to fill out a paper consent form.
The purchase of contact lenses is limited to customers who meet all of the following conditions. Please read the terms carefully and confirm your agreement. -
Customer Confirmation
□ Have visited an ophthalmologist and purchased it according to instructions.
□ Regularly visiting an ophthalmologist and is following the doctor's instructions. -
Requests to Customers
□ Please take regular check-ups at the ophthalmologist and follow the doctor’s instructions.
□ Please read the attached documents/manual carefully and follow the instructions for usage methods, lens care, and wearing time.
□ Please keep the documents/manual for reference when needed.
□ Wearing contact lenses may cause eye damage. If you feel any discomfort, please stop using the lenses immediately and visit the ophthalmologist.
☐ Returns and exchanges are only accepted for defective products.
☐ We do not provide contact lens fitting instructions or prescriptions. Please follow the guidance and prescriptions of your ophthalmologist.
☐ Any problems arising from not following the ophthalmologist's instructions or the attached documents/manual are the user’s sole responsibility.

☐ 本店除产品存在质量问题的瑕疵外，不接受退换货。
☐ 本店不提供隐形眼镜的佩戴指导或处方开具服务，请务必遵循眼科医师的指导与处方。
☐ 若因未遵守眼科医师指示或说明书使用方法而产生的问题，由佩戴者本人自行承担责任。
